I know there are some scholarly books on MUD/MUSHes, granted about fifteen years old at this point. Maybe we should dig into it? One of my other points I hope to address at OTW, is that maybe we (as an aspect of fandom, or a community with close ties) need to start bridging the gap and documenting this history.
